Request for Continuing Education Credit Hours

Contractors who attend a Blue Stakes of Utah 811 Damage Prevention Awareness Seminar are eligible to receive two continuing education credit hours with verification of attendance toward their contractor licensing as required by the State of Utah’s Division of Occupational and Professional Licensing (DOPL) as follows:

  • CONTRACTOR License Number ending with 5501, 5551 (2 core credit hours)
  • PLUMBER License Number ending with 5508, 5509, 5510, 5518, 5519 (2 professional credit hours)
  • ELECTRICIAN License Number ending with 5502, 5503, 5504, 5505, 5506 (2 professional credit hours)

Drinking Water & Waste Water Professionals who attend a Blue Stakes of Utah 811 Damage Prevention Awareness Seminar are eligible to receive CEU’s with verification of attendance toward their certification number as follows:

  • Drinking Water Professionals (0.2 CEU’s)
  • Waste Water Professionals (0.1 CEU’s)

If you attended a Damage Prevention Awareness Seminar in the last two years and did not request the credits at the time of attendance you may still be able to receive them by submitting the required information below to Blue Stakes at the following email address: msdept@bluestakes.org. Once all necessary information has been received and attendance has been verified, applicable credits will be reported to the appropriate organization.

  • Date & Location of Seminar Attended
  • All Applicable License and/or Certificate numbers
